29 CFR 1910.29(f)(4): Openings in stair rails. The employer did not ensure that no opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension.  On or about April 5, 2023, Unit Rig #405, employees were exposed to fall hazards because employer did not ensure that no opening in a stair rail system exceeded 19 inches in the following instances:  1)2-Open staircases on Directional Driller trailer 2)2-Open staircase on Company Man's Trailer 3)3-Open staircases on Safety Trailer 4)2-Open staircases Solid Control Trailer  This condition exposed employees to fall and trip hazards.
