29 CFR  1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws in the following area(s):  On or about August 23, 2023, on the west end of the outside of the Mud Engineer trailer, a strain relief for a 240v cord was observed as loose and not preventing pull from being directly transmitted to joints or terminal screws, exposing employees to electrical shock hazards.

29 CFR  1910.305(b)(2)(i): Covers of outlet boxes having holes through which flexible cord pendants passed were not provided with bushings designed for the purpose nor did the holes have smooth, well-rounded surfaces on which the cords could bear in the following area(s):  On or about August 23, 2023, on the west side of the Solids Control Trailer on an electrical box labeled "J", a flexible cord was observed passing through an opening without a bushing or well rounded surface, exposing employees to electrical shock hazards.

29 CFR  1910.305(j)(2)(v): A receptacle installed outdoors in a location protected from the weather or in other damp locations did not have an enclosure for the receptacle that was weatherproof when the receptacle was covered (attachment plug cap not inserted and receptacle covers closed) in the following area(s):  a) On or about August 23, 2023, on the west side of the Mud Engineer's trailer, a  240v water heater outlet was observed with a missing cover, exposing employees to electrical shock hazards.  b) On or about August 23, 2023, on the west side of the Directional Driller's trailer, a 120v outlet cover was observed as unable to close due to railing obstruction, exposing employees to electrical shock hazards.
