29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Facility: On or about April 6, 2024, an employee bypassed the fencing (locked gate) and entered the area of the Chem Coater while the line was running. The Chem Coater was not properly locked out nor tagged out. The Chem Coater drive was not locked out. This electrical disconnect to the Chem Coater was located about 30 steps away and in the drive (control) room. The employee was exposed to the following: 1. 2 drive arms (1 is for an applicator roll and 1 is for turn around roll) about 3 feet long and about 6 inches in diameter. The employee was about 12 to 18 inches from this hazard. 2. The employee that went into this area lifted up the physical guards to the all the rolls ( ie applicator roll, pickup roll and turnover roll). The employee was at most about 6 inches away from this hazard. 3. The edge of the coil of metal that is traveling through the machine. The employee was at most about 6 inches away from this hazard. (b) Facility: On or about July 12, 2024, an employee bypassed the fencing (locked gate) and entered the area of the Chem Oven while the line was running. The Chem Oven was not properly locked out. The Chem Coater drive was not locked out. The electrical disconnect to the Chem Coater was located about 30 steps away and in the drive (control) room. The employee was exposed to the following: 1. Bridle 2 A and 2 B are rotating rollers that are about 4 feet long, about 20 inches in diameter, about 48 inches in length and weight about 800 pounds The employee was about 3 feet away from this hazard. 2. 3 drive arms that are about 12 to 18 inches in length and are about 6 inches in diameter. The employee was about 3 feet away from this hazard. 3. The edge of the coil of metal that is traveling through the machine. The employee was at most 12 inches away from this hazard. 4. The air activated metal brake that has an opening of approximately 2-3 inches. The employee was at least 3 feet away from this hazard. IN THE ALTERNATIVE 29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) Facility, 125 McFann Road, Valencia, PA: The employer did not turn off or shut down machines or equipment using the procedures established for the machines or equipment. One instance of this violation occurred on or about April 6, 2024, when the Chem Coater Machine was not shut down using the company's written lockout/tagout procedures before an employee went beyond the locked fence gate while the machine was in production. (b) Facility, 125 McFann Road, Valencia, PA: The employer did not turn off or shut down machines or equipment using the procedures established for the machines or equipment. One instance of this violation occurred on or about July 12, 2024, when the Chem Oven was not properly locked out nor tagged out using the company's written lockout/tagout procedures before an employee went beyond the locked fence gate while the machine was in production.
