29 CFR  1926.451(g)(1)(vii):For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  Work site: 843 Lake Blvd, Lindenhurst, NY 11757.  On or about 08/20/2024, Employees working from a pump jack scaffold were not protected from falling approximately 18ft while installing exterior fascia on a two story residential home.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR  1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  Worksite: 843 Lake Blvd, Lindenhurst, NY 11757.  On or about August 20th, 2024. An employee was not protected from falling, while working on the roof of a residential condominium building approximately 30ft above the ground.   Note: The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR  1926.452(j)(2):Poles of pump jack scaffold(s) were not secured to the structure by rigid triangular bracing, or equivalent, at the bottom, top and other points as necessary.  Location: 843 Lake Blvd, Lindenhurst, NY 11757.  On or about August 20th, 2024. Employees were installing Siding to a residential home while working from a pump jack scaffold that was not secured at the bottom.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.  RCR Home Improvement, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.452(j)(2), which was contained in OSHA inspection # 1674654, citation number 1, item number 3 and was affirmed as a final order on 04/05/2024, with respect to a workplace located at 8 Nyack Drive, East Northport, NY 11731.
