29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  Employer was failing to protect employees from  hearing loss due to the lack of a hearing conservation program at the facility located 4657 G St. - Omaha, Nebraska. Employees are exposed to an 8-hour-time-weighted average noise levels that exceeded 85 A-weighted decibels (dBA) or an equivalent dose of 50% for the following employees:  :  a) In the Roll-In Room, a mixer operator was exposed to noise levels of 87.7 dBA or a dose of 72.8% as an 8-hour time-weighted average during a 393-minute monitoring period, on or about August 21, 2024. A zero increment is included for any time not sampled during the shift.    b) In the Roll-In Room, a mixer operator was exposed to noise levels of 87.9 dBA or a dose of 75.1% as an 8-hour time-weighted average during a 423-minute monitoring period, on or about August 21, 2024. A zero increment is included for any time not sampled during the shift.
