(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more.  a. On or about November 6, 2024, employee(s) engaged in commercial bakery maintenance activities at 4651 F Street, Omaha, Nebraska were exposed to fall hazards. The employee(s) were exposed to fall hazards while performing maintenance on elevated surfaces above 4 feet, without the use of fall protection. This violative condition was documented when employee(s) were working on the top of the Proof Box gear drive #2.  b. On or about November 6, 2024, employee(s) engaged in commercial bakery maintenance activities at 4651 F Street, Omaha, Nebraska were exposed to fall hazards. The employee(s) were exposed to fall hazards while performing maintenance on elevated surfaces above 4 feet, without the use of fall protection. This violative condition was documented when employee(s) were working on the top of the Proof Box gear drive #3.  c. On or about November 6, 2024, employee(s) engaged in commercial bakery maintenance activities at 4651 F Street, Omaha, Nebraska were exposed to fall hazards. The employee(s) were exposed to fall hazards while performing maintenance on elevated surfaces above 4 feet, without the use of fall protection. The employee(s) were exposed to fire and electrical hazards at the Tromp Line due to raceways in use not being properly secured.

On or about November 6, 2024, employee(s) engaged in commercial bakery production activities located at 4651 F Street, Omaha, Nebraska. The employee(s) were exposed to fire hazards due to the electrical equipment on or near the Tromp Line was not approved for the environment in which the employee(s) were working. Flour dust, which was present, was tested and found to be explosive on the Tromp Line.
