On or about October 28th, 2024, employees engaged in maintenance and oven lighting operations located at 4657 G Street, Omaha, Nebraska 68117, were exposed to fall hazards in that employees were working on top of the Blue Oven without receiving fall protection training.  Instances of failure include but are not limited to:   A.) An employee exposed to fall hazards when lighting the Blue Oven.  B.) Multiple employees exposed to fall hazards when performing maintenance on the Blue Oven.

On or about October 25th, 2024, employees engaged in oven lighting operations located at 4657 G Street, Omaha, Nebraska 68117, were exposed to electrical hazards in that the employer failed to assess oven lighting operations for the Blue Oven.  As a result of not conducting an assessment and providing employees with the proper PPE when performing this task,  an employee was shocked when attempting to light the Blue Oven.

The employee shocked was not provided with electrically rated gloves, electrically rated sleeves, or an ARC flash hood.
