29 CFR  1910.147(f)(1)(ii):When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2):  At a facility located at 25201 MO-78, Independence, MO, 64056, employees were exposed to amputation and caught in hazards.  The employer exposed the employees to these hazards by failing to develop and implement methods to ensure employees do not come into contact with moving machine parts while testing or positioning machine components associated with bullet submodule machines.

29 CFR  1910.219(c)(4)(i):Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:  At a facility located at 25201 MO-78, Independence, MO, 64056, employees were exposed to caught in hazards.  The employer exposed the employees to these hazards by allowing them to work near an unguarded projecting shaft end that did not present a smooth edge and that projected more than one half the diameter of the shaft.  The shaft was used with an electrical motor to power the slug takeaway conveyor on the Bullet Submodule line 2 machine.
