29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)  a) Worksite located at or near 411 North 9th Street, Philadelphia, PA 19123 - On or about August 21, 2025, the employer did not provide effective information and training to an employee exposed to hazardous chemicals during facade cleaning and restoration activities. An employee was exposed to hazardous chemicals including but not limited to the following:  -Ethyl alcohol, mineral spirits, and alkaline salts, as contained in Bare Brick Stone & Masonry Graffiti Remover.  -Hydrogen fluoride, phosphoric acid, glycolic acid, citric acid, and nonionic surfactant, as contained in Prosoco Sure Klean Heavy Duty Restoration Cleaner. -Hydrogen chloride, as contained in Prosoco Sure Klean 600. -Xylene, isophoronedialdimine, and ethylbenzene, as contained in Sikaflex 15 LM. -etc.

29 CFR  1910.1200(g)(8):  The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s):  a) Worksite located at or near 411 North 9th Street, Philadelphia, PA 19123 - On or about August 21, 2025,  the employer did not maintain and ensure that safety data sheets were readily accessible to employees performing facade cleaning and restoration activities and using products containing hazardous chemicals, including but not limited to the following:  -Ethyl alcohol, mineral spirits, and alkaline salts, as contained in Bare Brick Stone & Masonry Graffiti Remover.  -Hydrogen fluoride, phosphoric acid, glycolic acid, citric acid, and nonionic surfactant, as contained in Prosoco Sure Klean Heavy Duty Restoration Cleaner. -Hydrogen chloride, as contained in Prosoco Sure Klean 600. -Xylene, isophoronedialdimine, and ethylbenzene, as contained in Sikaflex 15 LM. -etc.

An employee was provided with and used PPE such as, but not limited to ANSI Standard A5 cut resistant gloves while performing work with products containing corrosive and hazardous chemicals, which did not protect the employee from chemical burns.
