29 CFR  1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.   a - 159 & 161 Red Lion Court, Atco NJ: Two workers and the company worksite Foreman were exposed to fall hazards, in excess of 9 feet 2 inches to the ground surface below, when using a portable aluminum extension ladder's damaged fly section to access and egress a residential structure's pitched roofs.  Condition observed on or about 04/22/26.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a- 159 & 161 Red Lion Court, Atco NJ: Two workers and the company worksite Foreman, performing sheathing installation and other related activities, were exposed to fall hazards in excess of 9 feet 2 inches when working and walking from a residential structure's pitched roofs while wearing full body harnesses whose attached lanyards lengths would not have prevented them from contacting the ground below. Condition observed on or about 04/22/26.  b- 159 & 161 Red Lion Court, Atco NJ: A worker, performing sheathing installation and other related activities, was exposed to fall hazards in excess of 9 feet 2 inches when working and walking from a residential structure's pitched roofs while wearing a full body harnesses whose attached lanyard was improperly connected to an anchor. Condition observed on or about 04/22/26.  Fast Builders Contractors Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1866761, citation number 1, item number 1 and was affirmed as a final order on February 2, 2026, with respect to a workplace located at Building 20 Holtz Way, West Windsor NJ.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
