An employee was using a scraper to clean a dryer chute. As the employee was cleaning the inside of the chute, the air lock blades amputated the employee's right middle, ring, and index fingers. The machine was running at the time.

An employee was adjusting a guard on a machine when his glove caught on the sharp edge of the guard. The metal guard partially amputated the employee's left middle finger.

An employee working in the sausage stuffing process area went to the storage area to retrieve casing for the sausage. The employee retrieved about 25 lbs. of casing using a stainless steel rolling cart. While pulling the cart back to her work area she lost footing and fell with the cart. The employee held onto the cart's handle and her fingers got caught between the cart and the concrete floor, amputating the left middle and ring fingertips.

An employee was packing hind feet in boxes when their artificial finger nail was caught in a segmented transfer conveyor. The employee sustained an avulsion and required surgical amputation of the left middle fingertip.

An employee was using a rag to wipe lubricant off the feed table of a flexo machine. The rag was pulled into the feed rollers, pulling the employee's hand with it. The employee sustained an amputation to the right index finger.

An employee in the deli department was cutting tomatoes with a mandoline when the mandoline amputated their little fingertip.

An employee had setup a table saw to cut a small piece of wood. While cutting the wood, the blade of the saw caught the employee's left index finger and partially amputated it at the first knuckle.

An employee was changing the battery on a forklift when her left middle and ring fingers were caught between the battery and the structure. The middle fingertip was amputated and the ring finger sustained a laceration.

On December 30, 2020, an employee was manually lifting a roll of film. The employee tripped and fell; the roll landed on their left hand. The end of the employee's fourth finger was amputated.

An employee was operating a gluer machine. The employee's left hand was caught in the feed section of the machine, resulting in the amputation of the hand.

An employee was operating a paper rewinder machine when he slipped and his fingers went into a nip roll. The employee sustained an amputation to the pinky fingertip, and two other fingers were injured.

During setup of a hydraulic hose cutoff machine, an employee was removing a piece of scrap hose when the machine activated and amputated the employee's right index fingertip.

An employee had just finished changing and adjusting the belt on a foundry standing grinder when the employee's left index finger made contact with the rotating wheel, resulting in a partial amputation.

An employee's right little finger was caught in a closing door. The fingertip was amputated.

An employee had been loading a safe with a forklift. The employee was adjusting the forks of the forklift when their finger got caught between the forks. The employee sustained an amputation to the left ring finger.

An employee was moving a set of rollers. The crossbar fell off the rollers and struck the employee's toe, causing its amputation.

An employee was cleaning flakes around a hopper/box. The employee's right hand came in contact with machinery under the hopper, causing injuries to the middle and index fingers. One joint of each finger was amputated.

On December 26, 2020, an employee reached under a chain conveyor to get a piece of wood that had splintered from a pallet. The conveyor had been stopped, but it advanced and the employee's left hand was pulled between it and a tension roller. The employee suffered crushing injuries to the hand and amputations to the left middle finger and thumb.
