An employee was backing up a powered industrial vehicle. Her left hand was caught between the vehicle and a rack, and the wrist was broken.

An employee was waiting for an operator to bring mail over to a mail sorting machine when she became pinned between the machine and a stack of pallets being pushed by a powered industrial truck (PIT). The employee suffered bruising and swelling on her hips, lower back, knees, and left side; a puncture wound to her left thigh from a machine screw; a crushed right hand with numbness and tingling; numbness to the left big toe; and a right wrist sprain.

An employee was using a skid loader to clean an area of loose debris (straw shed). He was lowering the bucket when the arms/bucket of the skid loader pinned his feet against the step of the loader. The employee sustained fractures to both feet and was hospitalized.

An employee was operating a reach truck. They were driving down an aisle in a warehouse when the equipment malfunctioned and lost throttle control. The reach truck struck a stationary rack. The employee sustained fractures to both lower extremities, which required surgery.

An employee was delivering a package. A dog bit the employee in the abdominal area, and the employee was hospitalized.

An employee was re-tensioning the pulleys on a conveyor belt in a shipping merge. He restarted the conveyor and his right index finger got caught in the belt, which pulled it into a pulley and amputated the top of the finger just past the fingertip. The pulleys were not guarded at the time of the incident.

An employee was loading a trailer. He was sitting on a pallet in the trailer to adjust and secure the load. When he dismounted the pallet, he fell approximately 4 feet to the trailer's metal floor, resulting in lacerations on both elbows and a fractured left hip that required hospitalization and surgery.

On August 22, 2025, an employee was working to remove a blockage of grain from a spout in the basement. The gate was manually closed from the floor above to stop the flow of grain to the leg. The gate closed down on the employee, causing the amputation of their left middle fingertip.

An employee was unloading packages from a package wall. When he picked up a package and turned to move it, another package on the wall fell. A 3-foot tube on the package struck the employee's right big toe, causing hospitalization with an open displaced fracture of the proximal phalanx of the toe.

An employee was performing deliveries when he felt light-headed and experienced cramps. The employee sustained a heat-related illness.

On July 23, 2025, at approximately 4:00 PM, a driver was on his route making deliveries when he started to feel ill from heat exposure. The employee sustained dehydration.

On June 29, 2025, at approximately 10:25 PM, a warehouse employee was moving product from one pallet to another. As she was turning, shrink wrap got caught on her leg, and she fell on the ground. The employee sustained fractures to her left arm, requiring hospitalization and surgery.

On June 27, 2025, at approximately 8:35 AM, an employee was operating a box maker machine when their left middle fingertip was caught by a chain and sprocket. The employee's fingertip was partially amputated without loss of bone or the fingernail.

On April 30, 2025, an employee exited their vehicle and were standing in an aisle when they were struck by a cherry picker. The employee sustained a right leg fracture.

An employee was on a dock, pushing a container full of mail on an incline. The container rolled backward and tipped over onto the employee's legs, causing him to fall and strike his head on the concrete floor. He sustained a laceration to his head and an injury to a tendon in his right leg, which required surgery.

An employee was using a standing forklift to store two pallets in an upper location and was lowering them into the final position when the upper pallet containing tires became unstable. The employee exited the forklift to adjust the pallet when it fell and the tires struck the employee. The employee sustained a fractured tibia.

An employee was taking a pallet of trash to the outside dumpster. Prior to unloading the trash, a piece blew away in the wind. The employee ran after the piece, tripped, and fell on the concrete. The employee sustained a fractured left arm near the elbow and scrapes and bruises to her nose and face.

An employee was using a pallet jack to move pallets when he lost his grip on the handle and fell to the floor. The employee sustained a fracture to his upper left arm.

On March 5, 2025, a package handler was moving packages off a slide to a belt and was lifting a heavy, irregularly-shaped box when it shifted. The box came down on their right hand and pinched a finger against the slide. The employee sustained a partial amputation to the tip of the finger.

At about 4:45 p.m. on February 11, 2025, an employee was attacked by a dog while delivering mail at a home. The dog bit the employee's hand. The employee was hospitalized and required surgery.

An employee was delivering mail to a residence when they slipped on black ice in a residential driveway and fell. The employee sustained a fractured right hip.

While dismounting from the cab of his tractor trailer, an employee slipped from the steps and fell to the ground. He sustained a dislocated left elbow, a broken left wrist, and a broken bone near the left elbow. He was was hospitalized and required surgery for the fractures.

On October 5, 2024, a crew was staging the last string of railcars to be loaded when the injured employee stepped forward and tripped on a railroad tie, causing his right foot to be on the rail. A staged railcar rolled forward and caught his foot between the wheel and the rail. The employee was hospitalized for amputations to toes.

On October 3, 2024, the injured employee was conducting a pre-trip inspection on their yard truck when a tractor trailer struck the yard truck. The injured employee was thrown several feet and was hospitalized with fractures to both legs and a brain injury.

An employee, who was operating a forklift, was entering a parked trailer when the trailer pulled away from the dock. The forklift began to roll off the back of the trailer, and the employee jumped off the forklift and landed on the dock. The forklift then fell and landed on the dock, pinning the employee's right leg, resulting in an ankle injury.
