An employee was operating a lift truck when it struck a stationary lift truck. His left thumb was pinched between the two lift trucks, resulting in a partial amputation.

A building engineer was inspecting a generator. As he closed the generator enclosure door, his right ring finger was caught between the door and the door frame. The fingertip was amputated.

An employee in the warehouse was working to pull a reel from under a rack when his finger was crushed between the reel and the rack. The employee sustained a fingertip amputation.

An employee was moving dies on a hydraulic pan brake closer together so he could bend pieces of sheet metal. The employee's left index, middle, and ring fingers were crushed in the machine, and all three fingers were amputated.

An employee was finishing up using a resistance welder on a vehicle. After finishing, he removed his gloves and picked up the resistance welding arm with his left hand on the hydraulic cylinder and his right hand on the handle. The button on the handle was activated and the hydraulic cylinder engaged, catching the tip of his left little finger and resulting in a fingertip amputation.

An employee was changing the location of bolts that held a metal plate on the top of a car wash conveyor. His thumb became stuck between a part of the equipment and a roller on the conveyor. He sustained a fracture to the right thumb just above the base knuckle, resulting in an amputation.

An employee was planing lumber on a jointer. While he was feeding the workpiece into the jointer, it hung up and he lost his grip on it. His gloved hand came into contact with the point of operation, and his left little finger was partially amputated near the first joint.

An employee was walking on top of a piece of equipment while engaged in repair activities. The employee stepped onto a rotating impeller shaft of a belt-driven exhaust fan and sustained a partial amputation to their right leg below the knee and amputation of their left hand.

An employee was working on a control line when it got jammed with steel strips. They went to unjam the machine and suffered a fingertip amputation to the left middle finger.

An employee was operating a steel rolling machine when their left fingertip became caught in the rollers, resulting in a fingertip amputation.

An awning was being moved when it fell and crushed an employee's left thumb against the roller it was about to be placed on. The employee suffered a partial amputation to the thumb.

On August 21, 2025, an employee was hitching a 20-foot flatbed trailer loaded with a tractor to a pickup truck. As she placed her left fingers near the hitch mechanism to check the remaining distance needed for coupling, the trailer jack dropped into place, contacting her middle and ring fingers. The hitch and ball then made contact with her middle finger. The employee suffered crush injuries to her middle and ring fingers and a fingertip amputation of the middle finger.

An employee was working to attach a safety handle to the cable portion of a pulley system designed to aid in the operation of a resistance welder. The tension on the cable was disengaged, then reengaged while the employee's left hand was on the top of the pulley system. The cable caught the tip of the employee's left little finger, causing an amputation at the first joint.

At about 8:00 a.m. on August 8, 2025, an employee was holding a metal coupon for another employee to use in demonstrating spot-welding technique to a third, student employee. A metal hole puncher was activated and crushed the employee's left middle finger, resulting in an amputation to the finger. The incident occurred during a training activity.

On August 6, 2025, an employee was placing cardboard into a cardboard compactor when his right middle and index fingertips were amputated. The employee was hospitalized for hand surgery.

A driver was doing a pre-trip inspection on a tractor when their fingers got caught in a fan belt and the tops of two fingers were amputated.

On July 19, 2025, an employee was filling a wheelbarrow tire with air when the tire exploded, resulting in severe injuries to their right hand including a partial amputation.

On July 25, 2025, an employee was lowering a vehicle on a lift. The employee's right index finger was caught in the lift and partially amputated.

An employee was operating a hoist and receiving the rope as it came into the hoist. Tape on a rope splice caught her glove and pulled her right hand into the capstan. The employee sustained crushing injuries resulting in amputations of the ring and little fingertips to the first knuckle.

An employee was conducting repair work on a dozer. They had to remove the tracks to make the repair and were preparing to put them back together when two track pads buckled and pinched the employee's left thumb. The employee's thumb was partially amputated.

An employee was moving a truck bed (200-300 lbs.) on a cart/carrier to the paint prep area. The truck bed slid toward him and his finger got caught between the truck bed and the carrier. The employee's fingertip was partially amputated.

An employee was holding a backup wrench on a torque machine when his left thumb was pinched between the wrench and the adjacent nut, resulting in amputation of the thumb tip.

An employee was installing an automobile lift at a client location and put the lift on a safety lock. The employee's right hand was caught in the lift and crushed, causing the amputation of the little finger at the last knuckle and all of his right fingers were fractured.

An employee was preparing a truck at a loading dock. She was placing the manual dock leveler device when her left middle finger became caught in the leveler. The employee's finger was partially amputated.

Two employees were working at the hydraulic shop refilling a square dump truck cylinder. While pushing the piston back on top, the cylinder fell onto the injured employee's little finger. The employee sustained an amputation of the finger to the distal knuckle.
