The employee released the dead man switch to stop the forklift and then dismounted the vehicle during the slide. His left foot/ankle was struck by the vehicle and a plastic pole protector as the vehicle continued to travel in the direction he exited. He suffered multiple fractures and a puncture wound.

At 4:15 p.m. on 7/18, an employee was cleaning a hot polymer glue press-pot using a scraper. The scraper broke, which caused both hands and her left forearm to come into contact with the hot glue. She sustained second degree burns to the palms and fingers of both hands and to the left forearm.
