EVERGREEN CONCRETE CUTTING INC
An employee was core drilling 1-inch holes into a concrete wall at a 45-degree angle downward. The employee was hand-turning the drill bit while it was attached to the drill and on a post mounted to the wall while using a file to sharpen the segments at the end of the bit. The set screw came loose, sending the drill and the bit free. Both the drill and the bit fell down the post and ripped the tip of the employee's left ring finger, resulting in a surgical amputation to the first knuckle.
