Final narrative
Employee was on the side of the derrick untying tubular when his fast line became caught up in his SRL. The employee was pulled away from the ladder. The SRL stopped and so did the anchor. Therefore, no fall resulted. The employee received contusions to the head, ribs, and chest area. He also suffered bruising to the brain and lungs and received six stitches to the forehead. He did not lose consciousness.

A crew was pinning two cross braces between substructure beams. As the outer cross brace was being pinned, the inner brace (6-inch square tubing, weighing 524 pounds) fell from the transport bracket, catching the employee's finger between the brace and the frame. The employee sustained a left index finger fracture.
An employee was setting up lights on opposite ends of beams. Vibrations caused a beam to slip and crush the employee's left index finger against another beam. The employee sustained an open fracture to the finger.
An employee was working with a catwalk, changing out a cable. After removing the skate cable, the employee and a co-worker were adjusting the skate to install the new cable. On September 12, 2022, an employee was on a metal roof, installing a standing seam and insulation. He lost his balance and fell toward the third floor slab, which was 10 feet from the roof. His fall protection caught him and left him suspended about 2 to 4 feet from the slab. He suffered a brain bleed and a spinal fracture in his lower back. He was hospitalized.
