On or about 1/8/15 at 12:10 a.m., an employee was working between two boxcars located on a private railroad when the cars began to move. The employee tried to get out from between the cars when one of the cars pinched his left foot, resulting in fractures.

An employee was walking on an uneven surface in the field when they tripped and fell on the ground. The employee sustained a fractured knee.

An employee was cleaning tandems with a hot water hose. While cleaning his left boot with the hot water, he sustained second and third degree burns on the left ankle.

An employee was driving a tractor up a trailer ramp when the tractor started going backwards and rolled over. The employee was struck by the tractor and suffered a pelvic fracture.

On December 4, 2023, an employee was helping to move a hopper car. He was climbing the stairs to go to the other side of the hopper car when he slipped. His right foot went under the hopper car's wheel and a toe was amputated.

An employee was working between railcars on an active railroad track to connect the airbrake. Another employee moved the railcars, striking and running over the employee resulting in amputation of both legs.

On March 25, 2022, an employee had just detached a string of empty rail cars away from some loaded cars. The string of detached cars was being taken ahead, and the employee began pulling the knuckle of a rail car into the open position. The string of detached cars then momentarily came back, and the employee was pinched between two cars. The employee suffered a broken right hip and was hospitalized.

At about 8:40 p.m., two employees (a groundsman and a switchman) were working in a rail yard, setting rail cars to be released from the site. The groundsman was watching the switchman connect an end-of-train device on the string of rail cars. While he was on the adjacent track, a locomotive on that track moved forward and struck him, knocking him over, injuring his right foot, and amputating his left leg above the knee.

On June 25, 2020, an employee was acting as a groundman to assist the locomotive driver in staging cars in the blast building. As the railcar was moving past a beam that is part of the building, the employee became pinned between the beam and the railcar and sustained a fractured pelvis.

An employee was adjusting the flat scratcher of a sweep cultivator when the metal tube of the scratcher slid down and caught the tip of the employee's left thumb between the pipe and C-clamp. The employee sustained a left thumb tip amputation.

A crew was building a scaffold inside a furnace. An employee was stabilizing the mudsill and jack as a 10-foot scaffold leg was being connected. The scaffold leg came in contact with the furnace wall, causing a piece of refractory brick to fall. It struck the right side of the employee's mid-back, causing a contusion.

American Sugar Refining Company

Employees were reinstalling the frame of a mechanical guard onto a raw sugar mixer. A chain fall was used to position the frame so it's weight was resting on the lower pedestal. The chain block was then unhooked, and an employee used a hammer to position the frame so the top bolt holes would be aligned. The bottom of the frame slipped off the pedestal and the frame fell two feet down onto the employee's right arm causing fractures to the radius and ulna.

An employee was tasked with identifying a conveyor system failure. When the employee opened the conveyor, a flash fire occurred. The employee sustained burns to the hands and face and was hospitalized.

An employee was operating package filling equipment when he attempted to loosen clumped sugar in the equipment piping and injured his right hand. He was hospitalized with fractures to four fingers on his right hand.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.
