An employee suffered two fingertip amputations while working at a plant.

An employee was threading an ID thread gauge into a part chuck and machine spindle. The thread gauge caught the glove on his right hand and pulled his hand into the spindle, crushing it. The employee was hospitalized with contusions and lacerations.

An employee had the left pinky fingertip amputated while setting up material on a lathe.

An employee was delivering mail when they tripped on a curb and fell to the ground, resulting in a kneecap injury.

An employee was leaving work when they tripped over a curb in the parking lot and fell into a wall, resulting in a fractured left humerus.

A nurse was visiting the home of a patient for requested treatment. She tripped on her gown while going up the steps to the property, falling and fracturing her left femur.

An employee tripped on a step while entering a house. The employee suffered a broken right femur and was hospitalized, requiring surgery.

An employee was walking up an escalator that was out of service when they tripped on a step, fell, and suffered a closed fracture to the right femur as well as a dislocated right patella.

On March 26, 2025, an employee was standing on a work table that was 23" high to abrasive blast the upper section of a pump. When he stepped off the table with his left leg, his left knee gave out and he fell to the ground, landing on his right hip. The employee sustained a displaced right hip fracture near the joint and was hospitalized.

An employee was pressure washing a suspended part when the part started spinning. The employee manually stopped the part from spinning when his hand was caught in the pressurized water blast. The employee was hospitalized with a laceration.

An employee was dumping a "self-dumping chip bin" (on the forks of a forklift) into a 3- or 4-foot-tall dumpster. The bin tilted forward. Thinking it was going to slide off the forks, the employee reached to grab the bottom of the bin to stop it. The bin then rotated back. The tip of the employee's left middle finger, from the first knuckle past his fingernail, was caught between the bin and the top of the forklift pocket frame. The fingertip was amputated just above the first knuckle.

An employee was operating a stand-up forklift in the warehouse when the forklift spun and the employee was caught between the forklift and metal shelving. The employee sustained multiple broken ribs.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
