An employee suffered two fingertip amputations while working at a plant.

An employee was threading an ID thread gauge into a part chuck and machine spindle. The thread gauge caught the glove on his right hand and pulled his hand into the spindle, crushing it. The employee was hospitalized with contusions and lacerations.

The employee went to step up on a platform that leads away from and around the work area. The employee tripped on the platform step (9 inches off of the floor) and landed on his right knee, breaking his femur.

An employee was operating a band saw when his right little fingertip was amputated.

An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.

An employee was operating a trim saw. While trying to free a jammed board she had just cut, she came into contact with the running saw blade, which amputated her right thumb.

An employee was using a shaper to cut wood when their right hand contacted the blade, resulting in a partial amputation to the right ring finger.

On March 26, 2025, an employee was standing on a work table that was 23" high to abrasive blast the upper section of a pump. When he stepped off the table with his left leg, his left knee gave out and he fell to the ground, landing on his right hip. The employee sustained a displaced right hip fracture near the joint and was hospitalized.

An employee was pressure washing a suspended part when the part started spinning. The employee manually stopped the part from spinning when his hand was caught in the pressurized water blast. The employee was hospitalized with a laceration.

An employee was dumping a "self-dumping chip bin" (on the forks of a forklift) into a 3- or 4-foot-tall dumpster. The bin tilted forward. Thinking it was going to slide off the forks, the employee reached to grab the bottom of the bin to stop it. The bin then rotated back. The tip of the employee's left middle finger, from the first knuckle past his fingernail, was caught between the bin and the top of the forklift pocket frame. The fingertip was amputated just above the first knuckle.

An employee was operating a stand-up forklift in the warehouse when the forklift spun and the employee was caught between the forklift and metal shelving. The employee sustained multiple broken ribs.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
