An employee was operating a forklift and navigating around a metal container. His left arm was severely lacerated when it was pinned between the forklift and the container.
The injured employee was operating a forklift to remove pallets of bananas from inside a container. A yard dog operator hooked up to the container and pulled it away from the dock as the forklift was exiting the container. The forklift fell between the container and the dock. As it was falling, the injured employee grabbed the overhead guard supports at the same time the forklift hit the steel ramp, amputating the right hand just above the wrist.
