On 2/19/15 at approximately 5:30 PM, an employee slipped on black ice in the parking lot, injuring her tail bone and requiring overnight hospitalization.
An employee was retrieving empty plastic totes from a material storage area. The totes weighed 7.5 pounds and measured 15 inches in width, 24 inches in length, and 7 inches in height.
