On July 22, 2024, at approximately 11:15 AM, an employee was moving an air conditioning unit on the roof when their right middle finger became caught between the bottom of the unit's frame and the roof curb frame. The employee sustained an amputation to the fingertip.

An employee was retrieving empty plastic totes from a material storage area. The totes weighed 7.5 pounds and measured 15 inches in width, 24 inches in length, and 7 inches in height. The employee tripped over conveyor rails and fell on his left side onto the concrete floor, resulting in hospitalization with a ruptured quadricep tendon in his right knee.

On July 18, 2024, at 1:09 a.m. in the receiving dock area an employee was pushing a trash can toward the compactor as a forklift was operating in reverse. The forklift struck the employee's left foot causing three toe fractures, including an open fracture of the big toe. The injured employee was hospitalized.

At about 3:00 a.m. on November 14, 2020, an employee was checking a molding machine containing seized-up material. As he opened the machine's door and looked down, the molding material gassed off and blew into his face. He suffered facial burns, as well as splatters on the neck and hands, and was hospitalized,

When they were returning to the front side of the assembly line, the employee stepped inside the frame of a finished goods cart and tripped, striking their head and right shoulder against the cart and the floor. The employee pulled his gloved hand away and suffered a fingertip amputation.

On May 16, 2025, an employee was performing iron work. He was sliding a 3/4" x 2' x 4' bent plate across another plate to position it for installation. The bent plate crushed his left middle finger between the two steel plates. The distal bone was fractured and required a revision amputation.

An employee was unloading product from a trailer. While walking out of the trailer, a powered pallet jack entered the trailer, slid on the dock, and pinned the employee's right leg against the trailer wall. The employee's leg was fractured.

On November 17, 2021, an employee was verifying the operation of a bulb crusher machine that had been restarted after a jam. The machine was jogged and pulled in the employee's right-hand glove. He suffered injuries resulting in the amputation of his right index and middle fingers.
