A worker was lifting a 42-pound part onto a shelf at chest height. The part fell onto his right-hand fifth digit and partially severed it at approximately the location of the cuticle.

An employee was moving a tie rod part from a cart to assemble into a larger part. The part slipped from his hands and when he tried to catch the 18 pound part his middle finger was crushed between the part and the table resulting in the amputation of the fingertip.
An employee was working behind a stack of steel I-beams (stacked 2 columns deep and 4 levels high on the floor) while a forklift was pushing the stack. When the forklift took off the second-to-last beam, the last one was pushed over, causing it to fall to the ground and strike the employee. The employee sustained a fracture to the right leg.
