The employee was installing a gear box at the Diedeshiem machine tool station (used in cutting metals for making toilet valves). The employee was using a hydraulic jack to align the gear box on the motor of the machine. His hand got stuck between the gear box disc and the motor case. His left hand pinky finger was amputated and his ring finger was also injured.

He had locked out the main control panel and shut off all air; he was aligning the cylinder with the yoke mounts to insert a new bolt that would secure the cylinder. The cylinder still had pressure in it, and it extended 2 inches when it was moved. His left hand was crushed between the cylinder yoke and the framework of a skip hoist bucket. He suffered a broken index finger and a laceration to the palm.
