At about 10:05 a.m. on September 13, 2022, an employee was repairing a machine with a loose cylinder. He had locked out the main control panel and shut off all air; he was aligning the cylinder with the yoke mounts to insert a new bolt that would secure the cylinder. The cylinder still had pressure in it, and it extended 2 inches when it was moved. His left hand was crushed between the cylinder yoke and the framework of a skip hoist bucket. He suffered a broken index finger and a laceration to the palm.
An employee was climbing a ladder when it slipped out from under him. He fell 5-7 feet to the floor below, suffering a head laceration, a broken wrist, and a possible spleen injury.
The employee was installing a gear box at the Diedeshiem machine tool station (used in cutting metals for making toilet valves).
