An employee was walking down stairs from the engine storeroom when she fell down the last five or six steps. The employee sustained injuries to her right hip and left ankle, and scratched her left elbow.
At about 10:05 a.m. on September 13, 2022, an employee was repairing a machine with a loose cylinder. He had locked out the main control panel and shut off all air; he was aligning the cylinder with the yoke mounts to insert a new bolt that would secure the cylinder. The cylinder still had pressure in it, and it extended 2 inches when it was moved. His left hand was crushed between the cylinder yoke and the framework of a skip hoist bucket. He suffered a broken index finger and a laceration to the palm.
