An employee was using an air nozzle attached to a hose when she tripped and fell over the hose and a T-handle. The employee broke her left ulna and radial bones in her forearm. She was hospitalized and required surgery to repair the broken bones.
An employee was clearing a jam from a process line when his right middle fingertip was amputated by the airlock. The machine was not locked out at the time.
