An employee was using an air nozzle attached to a hose when she tripped and fell over the hose and a T-handle. The employee broke her left ulna and radial bones in her forearm. She was hospitalized and required surgery to repair the broken bones.

As he pushed up, his left hand was placed between the fork and base of the dumpster. The base then lifted up, shifted, and came down and pinched his left fingertip. As he pulled his hand away, his fingertip was amputated.
