An employee was clearing a jam on an aspirator. The employee's left hand was caught in an airlock on the machine, which amputated the employee's ring, middle, and index fingertips.

As he pushed up, his left hand was placed between the fork and base of the dumpster. The base then lifted up, shifted, and came down and pinched his left fingertip. As he pulled his hand away, his fingertip was amputated.

An employee was exiting a moving shuttle wagon. As he landed, his leg made contact with uneven ground, causing an injury to his left tibia.
