An employee was clearing a jam on an aspirator. The employee's left hand was caught in an airlock on the machine, which amputated the employee's ring, middle, and index fingertips.

An employee was unloading corn from a dump bin attached to a skid loader. He was standing at the back of the dump bin and pushing it upward to get it to dump its contents. As he pushed up, his left hand was placed between the fork and base of the dumpster. The base then lifted up, shifted, and came down and pinched his left fingertip. As he pulled his hand away, his fingertip was amputated.
