An employee was operating a laser cutting machine in the laser shop. He was removing a piece of scrap sheet metal and placing it into the scrap/recycling bin when it hit the side of the bin and fell over the side, outside of the bin. While retrieving it, his right hand got caught on the edge of the scrap bin. The sheet metal piece fell to the floor and the rest of the sheet metal from the machine fell on his arm. The employee was hospitalized with a fractured arm and required surgery.

An employee working from a telescoping multiladder in a step ladder configuration fell from the ladder. The employee was hospitalized with a fractured right wrist.

An employee was fixing ceiling tiles while working from a ladder when they fell to the floor and sustained a hip fracture.

An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.

An employee was unloading materials from a flatbed trailer when they fell from the trailer and landed on the ground. The employee sustained a fractured left leg.

An employee was unloading appliances at a customer location for delivery. The employee was moving a water heater to be placed on the liftgate when he stepped backward and fell to the ground. The employee sustained rib and back injuries.

An employee was coming down a portable ladder when they slipped and fell to the ground, resulting in a right forearm fracture.

An employee was on an extension ladder descending from a roof when the ladder collapsed and lowered. The employee fell approximately 20 feet to the ground, resulting in fractures to their ribs and left wrist.

The injured employee was acting as a spotter for another employee that was using a forklift to move crane plates. The injured employee went to walk on top of staged bracing poles that had been stacked. The top layer of bracing shifted when stepped on, causing the employee to lose balance and fall. During the fall, one of the bracing poles dislodged and landed on the employee s left leg, causing a compound fracture.

An employee was working to remove an existing window from a customer's home. The employee was descending an extension ladder when they fell backward onto a concrete patio. The employee sustained head trauma and fractures to their shoulder, ribs, and a vertebra.

An employee was painting the exterior of a house when they fell approximately 12 feet from a ladder. They landed on the ground and sustained a fractured right ankle.

An employee climbed a ladder onto a roof, then stepped from new roof sheathing onto a tarped section of old roof, and fell to the ground. The employee suffered fractures to their pelvis, vertebrae, and the left wrist. The employee was hospitalized.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
