An employee was operating a laser cutting machine in the laser shop. He was removing a piece of scrap sheet metal and placing it into the scrap/recycling bin when it hit the side of the bin and fell over the side, outside of the bin. While retrieving it, his right hand got caught on the edge of the scrap bin. The sheet metal piece fell to the floor and the rest of the sheet metal from the machine fell on his arm. The employee was hospitalized with a fractured arm and required surgery.

An employee installing siding fell from a ladder breaking his left leg below the knee. The ladder was on the deck and slipped due to misty, wet weather conditions.

An employee was using a shepherd's hook to help position a jib crane. The hook slipped and the employee fell into an adjacent structure, suffering three broken ribs and a punctured lung.

An employee was using a water hose to clean the outside of a wastewater drain system when he fell into an area of the drain that was not covered. Hot water and chemicals caused first, second, and third-degree burns all over the employee's body. The employee was hospitalized.

An employee was cutting down a steel above-ground storage tank. He was on the roof of the tank when he fell 20 feet to the ground, resulting in a broken elbow and sprained leg. The employee was wearing a harness that was tied off at the time.

An employee was on an extension ladder descending from a roof when the ladder collapsed and lowered. The employee fell approximately 20 feet to the ground, resulting in fractures to their ribs and left wrist.

The injured employee was acting as a spotter for another employee that was using a forklift to move crane plates. The injured employee went to walk on top of staged bracing poles that had been stacked. The top layer of bracing shifted when stepped on, causing the employee to lose balance and fall. During the fall, one of the bracing poles dislodged and landed on the employee s left leg, causing a compound fracture.

An employee was working to remove an existing window from a customer's home. The employee was descending an extension ladder when they fell backward onto a concrete patio. The employee sustained head trauma and fractures to their shoulder, ribs, and a vertebra.

An employee was painting the exterior of a house when they fell approximately 12 feet from a ladder. They landed on the ground and sustained a fractured right ankle.

An employee climbed a ladder onto a roof, then stepped from new roof sheathing onto a tarped section of old roof, and fell to the ground. The employee suffered fractures to their pelvis, vertebrae, and the left wrist. The employee was hospitalized.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
