An employee was jogging a steel coil into a roll former to prepare the machine for a run. The coil started to slip, so he used his left hand to pull on it. His glove was pulled into the roll tooling, which crushed his left middle, ring, and little fingers. The employee sustained amputations to his left ring and little fingers.

An employee was struck on the shoulder by a forward moving forklift. He fell and the forklift ran over his left foot. A toe on the foot was amputated.
