An employee was using a compound miter saw to cut wood for packaging when the blade cut the back of the employee's right hand, requiring hospitalization. The saw was guarded at the time of the incident.
An employee was jogging a steel coil into a roll former to prepare the machine for a run. The coil started to slip, so he used his left hand to pull on it. His glove was pulled into the roll tooling, which crushed his left middle, ring, and little fingers. The employee sustained amputations to his left ring and little fingers.
Two employees were setting up and adjusting the tab position on a coil strip machine. The machine's press was actuated while one of them had his left hand in the press area. It crushed his index and ring fingers and amputated the tip of his middle finger.
