Employees went to a site for a water leak and had to remove the floor access hatch. While doing so, the door came off its hinge. They were attempting to reposition the hatch when the injured employee placed his fingers under the hatch to move it. The hatch then shifted on the broken hinge and dropped on his finger, resulting in the partial amputation of his left middle fingertip (no bone loss).
