An employee was using de-burring tools to remove burrs from a 25 inch metal diaphragm weighing 135 pounds. Two 2x2s were lying flat on a metal table and the diaphragm was placed on top. The employee had finished de-burring the top side of the diaphragm and was in the process of manually flipping it over when the 2x2 on the left side kicked out and the employee's left hand got caught between the fins on the diaphragm and the metal table, amputating the middle fingertip.

An employee was installing a valve on a compressor cylinder. The valve flipped and as the employee tried to stop it, his hands were caught in a pinch point causing amputation of the middle, ring, and pinky fingertips on the right hand. The employee was hospitalized and the fingertips were reattached.

Dresser-Rand Company

A maintenance employee was disassembling a machine. The machine was locked out and the alignment pins were installed to hold the part in place. After the last bolt was removed, the weight of the part sheared both pins and the part fell, striking the employee and breaking his ankle.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

Employee 1 was cleaning a paint gun using paint thinner when employee 2 started a lighter nearby and the cup of paint thinner ignited. Employee 2 dropped the cup and the ignited paint thinner spilled, causing burns on employee 2's arms, legs, hands, and face.

An employee was installing a part using an aerial lift and a ratchet. The lift rose and crushed his left thumb.

An employee was servicing a reciprocating air compressor when their left middle finger was caught in a belt motor pulley sheave while manually moving the belt. Approximately 1/4 inch of the employee's left middle fingertip was amputated.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.
