Final narrative
On July 24, 2015, an employee was adjusting a roll-forming machine when his hand was pulled into the running die. He broke his left thumb and sustained a left index fingertip amputation.
An employee was operating an uncoiler machine. He was inspecting the insert of the uncoiler when it caught his glove, pulling in his left hand. It sliced the thumb, causing an injury that required ten stitches and the insertion of a pin between the first and last joint.
A temporary employee was operating a power folder when the equipment was activated via the foot pedal. Two of the employee's fingers were then caught under the clamp, crushing the fingertips and resulting in a fingertip amputation.
