On Saturday, August 15, 2015, at approximately 2:00 p.m., an employee was operating a forklift at a roofing project jobsite. He was reversing the forklift when he made a sharp 90-degree turn with the boom extended, causing the forklift to overturn. He sustained injuries to his toes on the right foot and was hospitalized.

An employee was operating a sit-down type of material handling equipment (MHE). They were turning the MHE with the clamp attachment in the air when the MHE fell over onto its right side.
