An employee was standing on the back of his truck unloading water treatment chemicals into a tank at a municipal treatment facility. As the employee was climbing on top of a 330-gallon tote to reach another tote in the rear, he lost his footing and fell backward off the truck to the concrete surface below. The employee was hospitalized for a fractured collarbone, broken ribs, a possible concussion and a brain bleed requiring surgery.

An employee was fixing a battery-powered electric gate. After he reinstalled the battery, the gate actuated and his right index finger was caught in the chain and sprocket. The fingertip was amputated.

Employer reported that an employee showed symptoms of heat illness (or kidney function illness) and was evaluated and hospitalized with IV fluids.
