An employee was troubleshooting an issue with an elevator and ensuring the elevator cables were tracking properly on a sheave. To verify this, the sheave guard was removed and the elevator was operated slowly in the up and down directions. The employee became unsteady as the car moved up. He reached out to balance himself and his left hand contacted the sheave. The employee s left index and middle fingers were caught between the sheave and the cable, resulting in amputation.
