An employee was cleaning out rollers on a trash conveyor that was shut down. During cleaning, the employee removed tape from a roller. When the employee turned the conveyor back on, the tape was pulled back up and wrapped around the employee's left arm and pulled it into the conveyor, resulting in an upper arm fracture.

An employee was climbing a 6-foot A-frame ladder that was placed on a table to access and repair an oscillating fan mounted on the wall. The ladder was leaning against the wall and as the employee began to climb the ladder, the table moved away from the wall. The employee fell to the concrete floor and sustained bruised ribs and lacerations that required staples.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee was climbing down a ladder. She fell from about 3 feet up, grabbed storage racking but could not break her fall, and landed on the floor. She suffered an injury to the left knee, as well as cuts on the right middle and ring fingers. She was hospitalized.

She was at the bottom of the conveyor belt when the rug fell into the conveyor roller mechanism, jamming it. She was working to clear the jam when her right arm became caught in the 3.5-inch gap between the powered conveyor belt and the underside roller. The employee was hospitalized with six fractures, including her forearm, index finger, ring finger, wrist, and thumb, as well as a severed artery. The machine was not guarded or locked out/tagged out at the time.
