An employee was repairing a blower machine from a step ladder. While on the ladder and using a crowbar, the employee lost his balance and fell to the floor striking his head and back resulting in a concussion and back injury.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
