An employee was climbing a ladder to access and repair a piece of air compressor equipment when the ladder kicked off and came in contact with the structural surface. The employee suffered three non-displaced rib fractures and contusions.

The employee sprayed her hands with isopropyl alcohol (IPA) disinfectant. While using the alcohol and rubbing her hands, droplets of IPA fell to the floor. When the employee turned to walk away, her right foot slipped on the IPA droplets and she fell to the floor, landing on her right side and fracturing her right hip. The employee was hospitalized.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
