On September 22, 2025, an employee tripped and fell over a permanent fixture (angle iron) that serves as a parking block in the back of a facility. He suffered a fractured right hip and was hospitalized.

An employee was replacing the drive tire on a standup forklift. He used a hydraulic jack to lift the forklift and remove the worn tire. While installing the new tire, the forklift came off the jack and pinned his left hand under the drive tire. The employee sustained a broken left hand and required surgical amputation of the little finger.

An employee was training another employee on a hoist and heavy assembly operations for powered industrial vehicle manufacturing. One employee was lowering the power unit down when the injured employee's hand was caught and pinched between the power unit and a stationary piece of equipment. The employee's left thumb was partially amputated.

An employee was offloading equipment from a trailer when his thumb was caught between the trailer bed and the main frame. The employee was hospitalized with an injury to his thumb that required surgery.

On September 16, 2022, an employee was hoisting a product from a CNC mill. The employee's left index finger was caught between the product and a positioning bolt as they guided the hoist. The employee suffered an amputation to the fingertip without bone loss.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was operating a lift truck when it struck a stationary lift truck. His left thumb was pinched between the two lift trucks, resulting in a partial amputation.

An employee was disassembling a pressurized unit using a driver drill. When the unit was disassembled, it caused a kickback from the tube head, causing the drill to strike the employee's left forearm. They sustained a diaphyseal fracture to their left radius and ulna, which required surgery.
