Final narrative
On or about March 1, 2016, an employee was re-installing a pump that was approximately 5 feet long and weighed approximately 300 pounds. The employee's left ring fingertip was amputated when it was caught between the pinch point of the housing and the casting.
A panel was being lifted when an attached roller (approximately 20 pounds) slipped and fell about 2 feet onto the hard hat of an employee working below. The roller also struck the employee's shoulder and back. The employee was hospitalized and suffered a contusion to the shoulder/back.
An employee was sanding a piece of metal on a belt sander when his gloved hand was caught between the belt and the piece being sanded. The employee sustained a partial amputation to their right index finger.
