Sulzer Turbo Services Houston, Inc.

Employees were using a forklift equipped with four wire slings to move a 20-foot by 6-foot roofing panel from the work area to the laydown yard. The slings were threaded through the forklift's forks (two slings per fork), and the other ends were secured to the panel with shackles. The forklift was reversing through a bay door opening and the injured employee was applying pressure to a corner of the panel so it would fit through the opening. The load shifted, causing a sling to slide off a fork. The roof panel then fell onto the injured employee's left foot, resulting in a fractured ankle.

On February 10, 2025, an employee was operating a roller fan machine. The rollers pulled in the employee's hand, causing finger amputations.

Two employees were using a scissor lift to install a sign when a gust of wind knocked the scissor lift over. One of the employees fell to the ground, resulting in a broken left shoulder and left hip.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
