Final narrative
An employee was working on a modular unit approximately 4 feet from the ground. While walking to retrieve a tool, the employee stepped into an unguarded opening and fell 3 feet, suffering a fractured rib and pelvis.
An employee was coming out of a manhole onto some scaffold decking. The employee lost his footing, fell backward off the platform, and landed upright on both feet, resulting in fractures to two lower vertebrae.
