Final narrative
An employee was climbing a straight ladder when it slipped out from underneath him. He fell to the ground and fractured his right hip and ankle.
An employee was coming out of a manhole onto some scaffold decking. The employee lost his footing, fell backward off the platform, and landed upright on both feet, resulting in fractures to two lower vertebrae.
An employee was using a transport to move a ship component alongside a vessel. The employee was controlling the transport with a hand-held control, walking backward, watching the transport to make sure it had enough room to get by. At the same time, another employee was clearing gang boxes and material out of the transport's way using a forklift. After backing up with a load, the forklift began to move forward, and its back left tire struck the transport operator's right leg. The transport operator suffered a shattered tibia.
An employee was positioning a steel beam to be welded. As the beam was being lifted to turn over, it got caught on rollers. It came loose while the employee was trying to dislodge it, and his left hand was caught between the beam and a roller. He suffered amputations to the first and middle fingers just below the nails.
