An employee was working from a portable ladder when the employee slipped and fell. The employee's hand became caught on the metal garage door and was lacerated.
An employee was on the roof over a garage to install a gutter above the garage. He slipped and fell approximately 10 feet onto the concrete driveway. He sustained a fractured skull and a broken wrist. Fall protection was not worn at the time.
