An employee was removing a spandrel beam from a scissor lift. The employee lost hold of the spandrel beam and it fell on their left hand amputating the ring fingertip.

An employee was helping to place a pin into a shoe bolt on a frame that was being raised and lowered by a forklift when material slipped and smashed the employee's left middle finger. The finger had to be surgically amputated.

An employee was operating a wire brush handheld grinder to smooth out concrete. The grinder hit the side of a stair and kicked back, hitting the employee's left leg and causing a 2" deep and 4" wide laceration on his thigh.

An employee fell 18-20 feet through an uncovered hole in the roof while helping to install HVAC equipment. The employee was hospitalized for a facial fracture.

An employee was cleaning a spray gun. He had mineral spirits in the gun and was adjusting the spray tip when the gun shot the mineral spirits into the skin on one of his fingers. The finger was surgically removed and the employee was hospitalized.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was installing insulation in a wall when she tripped on a hole (12" deep) and fell, hitting her head on the concrete floor. The employee sustained fractures to her leg and wrist, and lacerations to her head. The employee was hospitalized.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
