An employee was repairing a hydraulic lift table and installed a locking chock on it. As he was working, the lift table collapsed, pushing the chock out and crushing his left arm. He sustained a fractured radius and ulna. The lift table was not locked out/tagged out at the time.

An employee was placing a box on a belt conveyor when the employee's left thumb was caught and amputated at the tip.

An employee's hand was on a spindle that controls flaps on a CNC saw machine. The flaps move up and down to guard and remove boards from the saw. The employee's left thumb was caught between the flaps, resulting in an amputation.

An employee began experiencing nausea and dizziness due to heat illness. The employee was hospitalized.

An employee was forming a part on a press brake. His left middle finger was pinched between the part being formed and the face/ram of the press brake, causing a partial amputation of the fingertip.

When the employee turned to pull the pallet away, the top two tiers of the pod fell on him. The employee sustained a fractured ankle, three fractured ribs, and fractured vertebrae.

An employee was loading a piece of wood onto a machine when the wood came out from under the guard and struck their arm, resulting in a fractured arm.

An employee was preparing to replace a belt on a fan drive assembly. He went to manually stop the fan and was caught by the belt. His hand was pulled into the machine and his left little fingertip was amputated.

An employee was cutting remanufactured lumber with a chainsaw when a board fell and struck his left hand, causing his left hand to move off the chainsaw. While he was moving his hand back, the chainsaw kicked back and lacerated the top of his left hand. The employee was hospitalized.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
