AMERICAN CAST IRON PIPE COMPANY

An employee was performing maintenance/electrical work inside a substation, and used a ladder to access the top of a panel box. The employee fell 8-9 feet from the top of the panel box to the ground. The employee was hospitalized with fractures to their ribs and a shoulder injury.

An employee was attending a pipe jack station where the edges of pipes are straightened with a mold and die. The employee was lowering and moving in a pipe when the die of the pipe jack hit the pipe, fell off, and hit the employee's left hand. The employee's left ring finger was crushed between the pipe and die, resulting in an amputation to the first knuckle.

An employee was preparing a stiff-leg pipe for installation on an overhead crane. The pipe fell and struck the employee's lower right leg, lacerating it. The employee was hospitalized.

An employee was trying to roll a crossed iron pipe back into the fixed metal pipe saddle. He lost his balance and as he tried to catch himself, his index finger was caught between the pipe and the pipe saddle crushing it. The fingertip was surgically amputated at the first knuckle. His middle and ring fingers sustained contusions.

An employee's hand struck a grinding wheel, suffering a fractured fingertip, a deep laceration to the index finger, and an abrasion on the middle finger. The employee was hospitalized.

An employee was drying and finishing vehicles when a vehicle door closed on their left middle fingertip and amputated it.

An employee was climbing down from a tractor trailer bed when they lost their footing and grabbed onto the roll-up door to stop the fall. The roll-up door then fell onto the employee's hand, resulting in a hand fracture that required surgery.

The employees began working to open the lid which appeared to be stuck due to cold weather. The injured employee pulled on one of the pivot arms that connect the lid to the main body of the trailer. As the lid began to open, the employee s right hand slipped down on the pivot arm and his index finger was caught between the pivot arm and the base of the pivot arm. The employee s right index finger was fractured and three nerves were severed. The employee was hospitalized.

An employee was repositioning a large casting with a chain. The chain slipped and landed on his hand, resulting in hospitalization with a fracture to his upper arm and a crushing injury to his thumb.

An employee was performing a machine set-up and had installed one set of holding jaws. His right index finger became caught in an air vise while re-clamping a part. The employee sustained a partial amputation above the fingernail.

Two employees were re-packing a butterfly valve. The valve was being held by a crane. An employee added air to the valve to close it. The valve moved and the other employee reached to steady it. His thumb was caught in the valve and the thumb tip was amputated.

An employee was preparing to install a production tree/master valve. The employee grabbed the bull plug and shook the production tree into place. The well blew out on the casing side. The pressure caused the production tree to fall over onto the employee, resulting in a fractured right leg.
